Click here to download the registration and waiver form required to play for any league. Without a signed wavier you will be unable to play. Children under the age of 18 require a parent or guardian's signature.
Creating an account on the Field House website will allow you to sign up for leagues, camps, or clinics online. You will only be able to sign up for these events online if you are a registered user.
You want to sign-up? It's simple. First click on the Register link in the navigation bar above or at the bottom of the right-hand column.
The user registration form will appear. Simply fill in your information and click the "Save" button at the bottom.
Log in and Sign-up! Make sure that once you are logged in you click the Edit link and complete your registration information. This will allow us to let you know of any upcoming events that may interest you.
